Individuals Ages 16-64 With High-Risk Health Conditions Now Eligible for Vaccination

​​​The easiest and most efficient way to make a vaccination appointment is at www.sccfreevax.org. That website also includes the most up-to-date information about who is eligible for vaccination and how to get vaccinated.

Residents without access to the internet can make vaccination appointments by phone for the County Health System vaccine sites through the Valley Connection Call Center at 408-970-2000.  

The Call Center is open Monday through Friday from 7am to 9pm and Saturday and Sunday from 8am to 4:30pm.  Assistance is available in English, Spanish, Vietnamese, and multiple other languages.

 California is now authorizing the vaccination of individuals ages 16-64 with high-risk health conditions.

The health conditions that meet the State's definition of high-risk are:
  • Cancer, current with debilitated or immunocompromised state
  • Chronic kidney disease, stage 4 or above
  • Chronic pulmonary disease,  oxygen dependent
  • Down syndrome
  • Immunocompromised state (weakened immune system) from solid organ transplant
  • Pregnancy
  • Sickle cell disease
  • Heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies (excludes hypertension)
  • Severe obesity (Body Mass Index ≥ 40 kg/m2)
  • Type 2 diabetes mellitus with hemoglobin A1c level greater than 7.5%
  • OR

    If as a result of a developmental or other severe high-risk disability one or more of the following applies:
  • The individual is likely to develop severe life-threatening illness or death from COVID-19 infection
  • Acquiring COVID-19 will limit the individual's ability to receive ongoing care or services vital to their well-being and survival
  • Providing adequate and timely COVID care will be particularly challenging as a result of the individual's disability
  • The State has also authorized vaccination for individuals who reside or work in a high risk congregate residential setting, such as an incarceration/detention facility, homeless shelter, or behavioral health facility. This includes all people experiencing homelessness as they are at risk of transitioning into congregate settings at short notice.

    Public transit workers, including airport and commercial airline workers, but not private airplane owners or workers, are also eligible for COVID-19 vaccination. They are at high risk for occupational exposure and maintaining continuity of transportation operations is critical for the community.
